These rights and responsibilities can help you and your family to learn about your treatment and make decisions about your care.

Our mission is to put you as our first priority by caring for you without regard to race, color, sex, national origin, religion, handicap, or disability. We will work with your doctor in providing medical treatment and assist you with other needs related to your care.
Our mission is very important to us. We accept the responsibilities that go with this mission. At the same time we need your help; patients and caregivers working together to achieve the best results from treatment.
The information on this page is being provided to you so that you will know what you should expect from our surgery center staff, and what they expect of each other. They can assist you and your family in learning about your treatment and help you make decisions about your care.
It is our policy at Black Hills Surgery Center of Wyoming, that all patients, in the event of an emergency, will be resuscitated and transported to the hospital of their choice.
We will perform services based not only on our patient’s safety, but also our team’s safety and best interests of both patient and staff. WE reserve the right to operate within the guidelines of our established policies as required by Medicare and AAAHC.
AS A PATIENT, you have the right to…
- Choose the facility and provider of your choice for health-related services.
- Know the name and professional status of those caring for you and expect continuity of care.
- Receive information from the physician and participate actively in decisions about your diagnosis, treatment plan, and prognosis.
- Have considerate and respectful care that respects you as a person as well as your values, beliefs, and culture.
- Personal privacy and confidential treatment. Written permission shall be obtained before medical records can be made available to anyone not directly concerned with your care.
- Have information about your care and treatment shared only with those responsible for your care.
- Receive care in a safe environment and be free from abuse or harassment.
- Leave the facility even against medical advice.
- Be informed of the continuing healthcare requirements following discharge from the facility.
- Examine and receive an explanation of a bill of a service, regardless of source of payment.
- Report any comments concerning your care and expect follow-up regarding your concerns.
- An interpreter or use of alternative communication techniques/aides as needed.
As A PATIENT, it is your responsibility to...
- Keep your appointment and notify the facility if you are unable to do so.
- Be accurate and complete, as much as possible, in giving your medical history.
- Read and understand all consents you sign. Please ask questions for clarification BEFORE signing consents.
- Carry identification with you.
- Let us know if you don’t understand any part of your treatment. Ask questions and take part in your healthcare decisions.
- Follow the treatment plan established by the physician, including instructions of nurses and other healthcare professionals as they carry out the physician’s orders.
- Arrange for a responsible adult to drive you home and stay with you for 24 hours after surgery.
- Pay your financial obligations promptly; if there is a hardship, let us know as soon as possible so we may help you.
- Treat staff and other patients with respect.
- Regard other patients’ medical information as confidential.
- Let us know when you are having pain or when your pain is not being managed.
- Respect Black Hills Surgery Center of Wyoming’s property and equipment.
- Inform staff about any advance directives that you have in place.
